Name That Game

In honor of the SNES B-day, I have chosen Super Mario World for this week's name that game. Why? One, because it's Super Mario World - the definitive game for this system - but also because it was the launch game for the SNES in 1991. But, the rules are different this time: I want you to name the other forms this game has taken as ports to other game consoles - what were the titles on their respective systems?
